Healthcare’s Role & Responsibility to Address Social Determinants of Health AJ Diamontopoulos, Kathryn JantzDenver Regional Council on Governments (DRCOG) and Rocky Mountain Health Plans (RMHP) are participants in the Center for Medicare and Medicaid Innovation Accountable Health Communities Model (AHCM). As part of this effort, they have cumulatively conducted over 90,000 social needs screenings of Coloradans since 2018. The results from the last year of these screenings present critical findings about the needs of Coloradans and the opportunity for healthcare to be critical partners in addressing those needs.

Achieving a 35% Reduction in Fall-Related EMS Calls Leveraging Private and Public Partnerships - The Nymbl and Lutheran Medical Center Case StudyJasmine Patriquin, Nathan EstradaOlder adult falls are the leading cause of injury in Jefferson County, Colorado. Lutheran Medical Center and Nymbl Science, in collaboration with a local fire department and senior living community, partnered together to provide an in-home fall prevention intervention. This intervention reduced the amount of fall-related EMS calls in a high-volume area and demonstrated the importance of in-home and community based fall prevention, leveraging multi-disiplinary partnerships.
